The Prevention & Wellbeing, Adult Mental Health Team is an award-winning Adult Social Care Provider in Cheshire West and Chester.
We work alongside a wide range of consultants, clinical experts and specialised agencies providing rehabilitation, treatment programmes, care planning, assessment, and long-term focus on recovery and overall wellbeing.

We seek an inspiring member of the team who can offer a ‘Can Do’ and flexible approach to the development and wellbeing of our customers.
Working across a range of mental health support functions, Senior Mental Health Support Workers provide short-term, intensive support to adult service users with mental health needs, enabling and empowering them to maximise their social, emotional, and physical independence in line with their individual care plan and the principles of mental health recovery. This involves ensuring that individual care plans are outcome focused and implemented and reviewed in a timely way, communicating any changes to the relevant internal colleagues and external multi-agency partners. Senior Mental Health Support Workers are required to work closely with individuals to develop onward pathways to appropriate services, community groups and third sector organisations to further enable independence and recovery.

What we expect from you
The successful candidate should have a natural yet professional approach to a wide range of circumstances and complex issues surrounding mental health including, forensic history, substance misuse, homelessness and many other challenging factors.
You will:
- Be committed to a person-centred approach to recovery and wellbeing
- Be enthusiastic about working as part of a dedicated team
- Be resilient and able to work under pressure and often alone.
- It is essential that you hold a Full UK driving license with Fully Comprehensive Insurance for Business Use.
